You will need to return the following:
– Certified death certificate
– Completed Beneficiary Statement (includes list of other requirements)
– Original policy if available
– Payment Option Election Form if applicable
Soon after filing the life insurance claim with Farm Bureau, you will receive a check (for proceeds under $10,000) or a checkbook with personalized checks (for proceeds $10,000 or more), along with information about our Living Tradition Account®. This is a free, interest-bearing account established for you as the beneficiary until you feel comfortable making decisions about the future.
